Ultrasound Sonography Treatment

Sonography is a technique which is noninvasive, Outpatient procedure used to visualize the internal organs and also helps in diagnosing any abdominal or pelvic abnormalities like a uterine fibroid, ovarian cyst, measuring the size of the growing follicle etc. Pelvic Sonography helps in visualizing the internal reproductive organs i.e. the uterus and ovaries. Pelvic sonography is performed by two methods-Transabdominal and Transvaginal.

In Transabdominal sonography a flat transducer is placed on the abdomen whereas in Transvaginal sonography a long and slender transducer is used in the vagina. If the woman relaxes her perineum and buttocks, then transvaginal sonography is not painful.

Pelvic sonography can be performed on any day of the menstrual cycle. It could also be performed when the woman has vaginal bleeding.

Ultrasonography and ART

Follicular sonography is a serial sonography performed to know about the number and size of follicles (woman’s eggs) that mature and get released during ovulation. On the 2nd day of the periods a TVS helps in assessing the woman’s baseline antral follicle count. From day 8 of the menstrual cycle, the size, number of the growing follicles and the endometrial lining of the uterus is measured. This is generally repeated on every alternate day until ovulation occurs. This is very useful for an infertile woman who is undergoing ART treatment cycle – in IUI cycles, in IVF and ICSI cycles.

In a natural cycle, to improve the chances of pregnancy, the couple is advised to have sexual intercourse around the time of ovulation. In an IUI cycle the dose of the hormonal injections are tailored depending on the follicular response in the woman. Also the IUI is timed with ovulation i.e. 36-40 hours after HCG injection. In an IVF and ICSI cycle the dose of the hormonal injections for the follicular stimulation are adjusted to obtain optimal maturation of the growing follicles. The oocytes are recovered 36 hours after the HCG injection as the oocyte would mature at that time. Ovum pickup for recovering the oocytes is done under sonography guidance. Embryo Transfer is also done under Sonography guidance. Follicular response varies in every cycle and hence it needs to be repeated in every cycle treatment.
